HEALTH SCREENINGS OCTOBER 24, 25, 26
The Massachusetts Department of Public Health Regulation requires students be screened at Chelmsford Public Schools at certain intervals.In elementary school, your child will be screened for:
Hearing: Grades *K, 1, 2, 3
Vision with stereopsis: Grades *K, 1, 2, 3
Visual acuity: Grade 4 Height/Weight Grades 1, 4
*Kindergarten Upon entering kindergarten ,or within 30 days of the start of school, the guardian shall present to school the Health Care Provider documentation from within the previous 12 months that the child has passed a vision screening. If your kindergarten child is missing a 2016 hearing or vision screening, it will be done at school.
Please make sure your student brings or wears their prescribed eyeglasses at school for the vision screening.
If you choose to have your child opt out of any of these screenings, parent/guardian must send a written request to exclude the student from participating. Please send this signed note to Amy Regan, Byam School Nurse or via email to email@example.com no later than October 20.

NOTES FROM THE NURSE
A Message for current Grade 3 Parents In accordance with Massachusetts General Law (Chapter 71, section 57) and related amendments and regulations (105 CMR 200.000-200.920), Chelmsford requires all 3rd grade students to have a current physical on file in our health office. Grade 3 students should have a physical dated January 2016 to December 2016 and a copy needs to be submitted to the health office by December 2016.
